C4 has been a leader in delivering training, consultation, and resources for Projects for Assistance in Transition from Homelessness (PATH) providers and outreach workers for over a decade. Working with individuals, families, and youth experiencing homelessness is at the heart of our work. We are committed to ensuring equitable access to safe, stable, affordable housing and services for all.
Our staff have led federal, state, regional, and local projects to identify and promote evidence-based and promising practices and provide training, technical assistance, and consultation to support outreach workers and improve housing and homeless services.
Our team includes service providers, trainers, researchers, and people who have experienced homelessness, mental illness, substance use, and trauma. Our work is grounded in our experience providing outreach, shelter, housing, mental health and addiction services, health care, and Continuum of Care planning and implementation. People with lived experience are included in all aspects of our work to ensure real-world expertise is embedded in all we do.
