When people experience substance use, mental health challenges, and housing instability, having options for high quality, culturally responsive services and supports is essential. Many communities have limited health and human service options and are often unprepared to meet the needs of all people.

Since 2006, C4 Innovations has collaborated with local and national partners to identify gaps, design solutions, and implement quality programs that support recovery, wellness, and housing stability.

C4 is a community of passionate, mission-driven, multidisciplinary subject matter experts with lived, living, and other professional knowledge, trainers, technical assistance specialists, clinicians, evaluators, and researchers. Our team brings deep expertise in assessing and meeting the needs of people across the lifespan from the earliest developmental years to transition-age youth and adulthood. We know from our own experiences and those of our partners, colleagues, families, and friends that with the right supports at the right time, people can and do recover and that housing stability is possiblefor all people

Our work with services and systems focuses on community-informed, person-centered, culturally responsive approaches so everyone gets what they need to achieve health and wellness. We focus on supporting people, communities, and systems to overcome challenges and stigmas related to mental health, substance use, housing instability, and homelessness. Many factors can make it even harder for people to get the support they need, including age, economic status, lack of transportation, racial or cultural factors, gender, sexuality, and disability. As a result, we work to identify and reduce barriers unique to each community. We work with our partners to:

Expand access to prevention, early intervention, treatment, and recovery supports.

Spanning substance use and mental health needs across the continuum and tailoring support for youth, adults, families, peers, and communities, we work to create and expand service options, choice, and access to recovery and well-being.

Improve systems and approaches for supporting people who are at risk of housing instability or homelessness.

Our team knows how to engage individuals, families, and youth, assess and enhance systems, and expand access to and quality of housing supports.

Facilitate change processes and transformation initiatives.

Our innovative, collaborative approaches spark learning and transformation and engage your staff, partners, volunteers, and board members.
